Fertility Preservation

What is Fertility Preservation ?

Fertility preservation involves techniques and strategies to safeguard reproductive potential for individuals who may face infertility due to medical treatments, age, or other factors.

Here’s an overview:

Purpose and Indications

Fertility preservation is often considered for:

  • Cancer Patients: Before undergoing treatments like chemotherapy or radiation, which can damage reproductive organs.

  • Autoimmune Disease Treatments: Such as lupus, requiring therapies harmful to fertility.

  • Age-Related Fertility Decline: Women wishing to delay childbearing.

  • Genetic Conditions: Those at risk of premature ovarian failure or other fertility issues.

  • Gender-Affirming Treatments: For transgender individuals before hormone therapy or surgery.

Techniques - For Women

1. Egg Freezing (Oocyte Cryopreservation):

  • Eggs are harvested, frozen unfertilized, and stored.
  • Suitable for women who want to delay

2. Embryo Freezing (Embryo Cryopreservation):

  • Eggs are fertilized with sperm to create embryos, which are then frozen.
  • Requires a partner or sperm donor at the time of preservation

3. Ovarian Tissue Freezing (Ovarian Tissue Cryopreservation):

  • A portion of ovarian tissue is surgically removed and frozen.
  • Can be reimplanted later to restore fertility or hormonal function.

4. Ovarian Suppression:

  • Medications like GnRH analogs are used to temporarily shut down ovarian function during chemotherapy.

Techniques - For Men

1. Sperm Freezing (Sperm Cryopreservation):

  • Sperm is collected, frozen, and stored.
  • Commonly used before treatments that may affect fertility, like chemotherapy or radiation.

2. Testicular Tissue Freezing:

  • Tissue from the testicles is frozen for prepubescent boys who cannot produce sperm yet.

Emerging Techniques

1. In Vitro Maturation (IVM):

Immature eggs are harvested and matured in the lab before freezing.

2. Ovarian Transposition:

Ovaries are surgically moved away from radiation treatment fields to preserve function.

Process

1. Consultation:

Initial evaluation and discussion with a fertility specialist.

2. Stimulation (for women):

Hormonal treatments to stimulate egg production.

3. Collection:

Retrieval of eggs, sperm, or tissue.

4. Freezing and Storage:

Cryopreservation and storage in specialized facilities.

Benefits and Risks

Benefits:

  • Preservation of fertility potential for future use.
  • Allows individuals to undergo necessary medical treatments without sacrificing the possibility of biological children.

Risks:

  • Procedures can be physically and emotionally taxing.
  • Not all preserved material will survive thawing or result in a successful pregnancy.
  • Costs can be high, and insurance coverage varies.

Success Rates

Success rates vary depending on the technique used, age at the time of preservation, and underlying health conditions. Generally, younger individuals and those with higher quality reproductive material have better outcomes.

In summary, fertility preservation offers valuable options for individuals facing potential infertility, providing hope and flexibility for future family planning.
